Monteith's official cause of death was mixed drug toxicity, involving intravenous heroin use combined with the ingestion of alcohol
Investigators at the scene found drug paraphernalia in Monteith’s room, including “a spoon with drug residue and a used hypodermic needle, along with two empty champagne bottles and glasses.”

GLEE's Cory Monteith Dies at 31
First reported by the Canadian media, and then confirmed by the Vancouver Police in a press conference this evening, Glee star Cory Monteith was found dead at age 31 on Saturday.
Canadian media initially has reported that it was a suspected drug overdose on Sat at Vancouver's Fairmont Pacific Rim hotel. The Vancouver Police held a press conference at 12:30AM EST, 10:30PM Vancouver time to announce the sad news.

Monteith had checked into the room on July 6th and was due to check out today. He was reportedly alone in the room after having gone out with friends, and was discovered by the hotel's staff upon missing his checkout time.
